I see these constantly, and every one of them is costing people opportunities:

The cropped group photo. Someone else's shoulder is still in the frame. It screams "I didn't care enough to get a real headshot."

The vacation photo. You look great in Maui. But sunglasses and a Hawaiian shirt don't belong on a professional profile.
